1. After Your Hospital Stay

    We at Inova understand that care does not end after a hospital stay. We offer a range of services to assist with medical and personal care, rehabilitation, special equipment needs and living arrangements. Medical House Calls The Inova Medical House Calls Program is especially designed to bring primary care to homebound patients who are 65 years of age or greater. Our healthcare team – including doctors, nurse practitioners and social workers – offers expert, comprehensive care for patients who have difficulty leaving the home.
  2. Outpatient Rehabilitation

    Outpatient rehabilitation includes three categories of service: acute (during or immediately following hospitalization), sub-acute (short term), and general outpatient services. Check the Rehabilitation Services section of our Web site for more detailed information. Acute Rehabilitation Acute rehabilitation following an illness or accident begins in the hospital or in a rehabilitation center once a patient is medically stable. Within this individualized and highly monitored setting, a patient receives several hours of therapy each day, including physical and occupational therapy.

    The following services are available to Inova patients 65 and over. Currently these programs are available only at Inova Fairfax Hospital.   4. Geriatrics Services

    As part of the Geriatrics Medicine Service at Inova, geriatrics clinicians work closely with a patient’s physicians, nurses and therapists to maintain a safe environment, prevent complications and preserve function and dignity. In addition, the team works to improve comfort and quality of life, identify potential age-related issues, and keep communication open and coordinated among team members and family members.

    Denise Mohess, MD, is Chief of Geriatrics at Inova Fairfax Hospital Tell Us a Little Bit About the Help Program. The goal of HELP, which stands for Hospital Elder Life Program, is to maintain function for the elderly while they’re at Inova Fairfax Hospital. When elderly people are in the hospital they are at risk for loss of strength and agility, as well as confusion and delirium. HELP works to ensure that these patients leave the hospital in as good or better shape than they were when they went in. One of the most special things about HELP is that it’s all done by volunteers.
  7. NICHE

    NICHE's collaborative nurse-driven approach to senior care draws on the unified expertise of an elderly patient's entire team of caregivers. Using NICHE guidelines, nurses, physicians, pharmacists, rehabilitation therapists, social workers, dietary staff and others work together to provide the most coordinated, effective care possible. NICHE develops expertise among clinicians by offering specialized geriatrics education in topics critical to the care of seniors, including normal changes of aging, pharmacology, nutrition, and cognition.
