Pre-Op Information Before Your Pre-Op Interview Fill out your online medical history using our One Medical Passport system:Online Medical History - One Medical Passport Interview: Before your surgery, one of our nurses will call you to conduct a telephone interview. During your interview a registered nurse will verbally inform you of your Patient Rights and Responsibilities. If this interview does not occur before your scheduled day of surgery your procedure may need to be rescheduled. Insurance Before the date of your surgery you will need to provide us with insurance information. Some insurance companies require pre-certification before surgery. It is your responsibility to ensure that all appropriate paperwork, provided by your surgeon, has been completed to allow our Business Office to obtain pre-certification from your insurance company.
