Physicians Referring physicians may speak with members of the lung transplant team 24 hours a day, seven days a week. See Details of Contact Information During Business Hours New Patients – When You Schedule When you schedule an appointment, in addition to your name, address and phone numbers, you will be asked to provide the following information: The names and phone numbers of your referring physician, your primary care physician and any specialists you are seeing. The names and phones numbers of your next of kin or emergency contact. Your insurance information, including the 800 number for provider services. Please identify any special needs that you or a family member has who is coming with you. Services are available for patients with hearing, vision or mobility needs. Upon request, interpreters are available for non-English speaking patients or family members.
